1. Schedule: The Win A Year's Supply of Vitamins Sweepstakes ("Sweepstakes") begins at 12:00 AM Central Time ("CT") on May 1, 2008 and ends at 11:59 PM CT on June 26, 2008 ("Sweepstakes Promotion Period").
2. How to Enter: You may enter the Sweepstakes by visiting http://sweepstakes.botanicchoice.com ("Web Site") and completing and submitting the online entry form as directed. Limit one (1) online entry per valid email address, per eligible person. Multiple Entries in excess of the stated limit by the same person will be disqualified. Information required for entry (collectively "Entry Information") consists of full name (first and last name), complete address (no P.O. Boxes), phone number, and email address. Incomplete or inaccurate online entries are void.
In the event of a dispute over the identity of an entrant, the entrant will be declared made by the natural person who, at the time of entry, is assigned to the email address (the "Authorized Account Holder"), according to the Internet service provider, online service provider or other organization that is responsible for assigning email addresses for the domain associated with the submitted email address. An entry will be deemed submitted by the Authorized Account Holder of the email address at the time of entry. Automated, programmed, robotic or mechanically reproduced entries will be disqualified. All online entries must be submitted during the Sweepstakes Promotion Period. Once submitted, all online entries become the exclusive property of Indiana Botanic Gardens, Inc., dba Botanic Choice, ("Sponsor") and will not be returned. Any attempted form of entry into the Sweepstakes other than as described herein is void. Entries that are in violation of these Official Rules will be disqualified.
3. Official Rules: For a copy of these Official Sweepstakes Rules, either (i) send a self-addressed, stamped envelope ("SASE") to Win A Year's Supply of Vitamins Sweepstakes "Rules", Botanic Choice, 3401 W.37th Ave, Hobart, IN 46342 or (ii) visit the http://sweepstakes.botanicchoice.com Web Site.
4. Eligibility: The Sweepstakes is open to legal residents of the United States (excluding Puerto Rico and the U.S. Virgin Islands) and the District of Columbia who are 21 years of age or older as of May 1, 2008; living in the United States or the District of Columbia; and have access to the Internet. Entrants by groups, corporations or organizations are not eligible to enter this Sweepstakes. Employees of Sponsor, Indiana Botanic Gardens, Inc, their advertising and promotion agencies, their respective parent companies, subsidiaries, affiliates, officers, directors, shareholders, board members, agents, service providers, employees and their immediate families (spouses, parents, siblings, children and each of their respective spouses), and/or persons living in the same household as such persons, whether or not related, (collectively, the "Sweepstakes Entities") are not eligible to enter or win this Sweepstakes.
5. Prize/Odds of Winning: Eight Grand Prize Winners will each receive their choice of products available from Botanic Choice, with a List Price Value of up to $500, which shall be delivered to U.S. addresses only. The Approximate Retail Value of Prize is $500.00. All costs, taxes, fees and expenses associated with any element of prize not specifically addressed herein are the sole responsibility of the Prize Winner. All federal, state and local taxes on a Prize are the Prize Winner's responsibility. The specifics of all elements of the Prize shall be solely determined by the Sponsor. Prize cannot be transferred. Prize must be delivered in no more than 4 (four) deliveries, paid for by Sponsor, and prizes must be redeemed by April 30, 2009. Merchandise is subject to availability. Sponsor reserves the right to substitute prize, in whole or in part, with a prize of equal or greater value. Any depictions of the prize are for illustrative purposes only. Odds of winning the prize depends on the number of eligible entries received.
6. Drawing and Notification of Winners: Drawings will be held weekly on or about May 8, 2008, May 15, 2008, May 22, 2008, May 29, 2008, June 5, 2008, June 12, 2008, June 19, 2008 and June 26, 2008 ("Drawing Dates"). Prize Winners will be randomly selected by the Sponsor in a drawing from among all eligible Entries submitted to date. The Sponsor will notify the winners by phone within approximately one (1) week after the Drawing Dates. If potential winners are notified by phone he/she must respond within five (5) business days of notification of winning or an alternate winner may be selected. Decisions with respect to the Prize Winners and other matters pertaining to the Sweepstakes shall be final. If potential Prize Winners cannot be contacted after reasonable efforts have been made, if potential Prize Winners are found to be ineligible, if potential Prize Winners fail to comply with these Official Sweepstakes Rules or any of the requirements, and/or if any prize or prize notification is returned as undeliverable, potential Prize Winners will be disqualified and an alternate potential Prize Winner shall be selected from the remaining eligible entries, at Sponsor's sole discretion.
7. Redemption and Verification: Prize claim is subject to verification and compliance with these Official Sweepstakes Rules. In the event of noncompliance with these redemption and verification requirements, the Prize will be forfeited and an alternate Prize Winner may be selected by the Sponsor.
8. General Conditions: The Sweepstakes is subject to these Official Rules and all federal, state and local laws and regulations (collectively "Rules and Laws"). If, for any reason, the Sweepstakes can not occur as originally planned or is compromised in any way, Sponsor, at its sole discretion, reserves the right to cancel, terminate, suspend or modify the Sweepstakes in whole or in part and conduct a random drawing to award the prize using all salvageable eligible entries received, if any, as of the end date of the Sweepstakes Promotion Period.
9. Privacy and Publicity: Unless prohibited by law, entry in the Sweepstakes and/or acceptance of a Prize constitutes permission for the Sweepstakes Entities to use worldwide and in perpetuity Prize Winner's names, address (city and state), images, likeness, biographical information, voices and/or video for trade, advertising, publicity and promotional purposes in all media now known or hereafter discovered, including without limitation, the World Wide Web (i.e., http://www.botanicchoice.com or http://sweepstakes.botanicchoice.com, ), television and/or radio, without notice, review, approval or compensation. Prize Winners agree to execute specific consent documents to such use if requested to do so.

Disclaimers: By entering and participating in this Sweepstakes, all entrants, Prize Winners, and his/her family members, agents, and employees release and hold harmless Sponsor and each of the Sweepstakes Entities, as well as the respective officers, directors, agents, employees, shareholders and board members of the foregoing entities and all others associated with the development and execution of this Sweepstakes from and against any and all actions, claims, and/or liability with respect to or in any way arising from participation in this Sweepstakes and/or acceptance or use of the prize(s), including liability for personal injury, death, damages or loss.
